PLEASE READ INSTRUCTIONS CAREFULLY

1. Your registration will be complete after you submit this form AND send in your payment by check. Checks are due within FOUR DAYS of submitting your online registration.

2.  Please write a separate check for each club to facilitate accounting. Make your checks payable to Anne Hutch PTA and send to:  Anne O'Connor, 64 Highland Ave. Eastchester, NY 10709. Write your child's name and club name in the memo line.

3. Please note that some clubs are restricted to certain grade levels.

4. You may register one child for multiple clubs on this form (one for each day), but please fill out separate registration forms for each of your children.

5. After registering, if you later decide you want to add another club, please fill out a new form with only the new club selected. Your child will already be on the roster for the clubs you selected previously.

6. All clubs are first come, first served (in the order that online registrations are submitted).  Clubs that are full will show a waitlist option.

7. If a club is full, you may sign up for the waitlist. Space may become available for a number of reasons, including people mistakenly registering twice because they thought their form didn't go through, or not sending in their payment. In some cases the teacher may be able to accommodate some additional students in the class or we may be able to open an additional section. No need to send payment when waitlisted, until you are notified your child is admitted.

8. If you are waitlisted for a club and are not admitted, you will be given the chance to register for clubs that have openings after the registration period ends.
